*Beginning January 1, 2005 California law requires the use of secure prescription paper for all prescriptions. Senate Bill 151 mandates that all written controlled substance prescriptions (schedules II-V) must be on security prescription forms. These security prescription forms are required to have at least 7 specified security features and can only be produced by printers that have received approval from the California State Board of Pharmacy. Prescriber's name must be preprinted on the form along with their address, phone number, license number and DEA authorization number. A fax or scanned copy of the prescriber's license # and DEA # must accompany all orders. |
Here's how to order:

- Select the type of forms you want.
- Enter the required information for your name, license number, DEA number, address, etc.
- Press submit and you will see an on-line version of your form. If correct, add to your shopping cart.
- To order, click the link for shopping cart. If this is your first time you'll need to register.
- Complete the information required in the shopping cart and complete check-out.
- Following check out, fax or e-mail a scanned copy of your license and DEA authorization to
